Sukhada Solace for Elderly Palliative Care Policy on Admission.

To ensure that Seniors staying to our facility receive comprehensive and compassionate palliative care, focusing on relieving suffering, improving quality of life, and supporting Seniors and their families throughout the care continuum.

Scope: This policy applies to all Seniors staying to our facility, regardless of their diagnosis, prognosis, or treatment goals.

Policy Statement:
1. Screening and Assessment: All Seniors staying to our facility will be screened for palliative care needs using a standardized tool.
2. Palliative Care Consultation: Seniors identified as having palliative care needs will be referred to Sukhada team for consultation and development of a personalized care plan.
3. Comprehensive Care Plan: Sukhada team will work with the patient, their family, and other healthcare providers to develop a comprehensive care plan that addresses the patient's physical, emotional, social, and spiritual needs.
4. Communication and Collaboration: Sukhada team will communicate and collaborate with other healthcare providers, including primary care physicians, specialists, and hospital staff, to ensure that Seniors receive seamless and coordinated care.
5. Family Support: Sukhada Team will provide emotional and practical support to Seniors' families, including counselling, education, and respite care.
6. Quality Improvement: Sukhada team will participate in ongoing quality improvement initiatives, including monitoring patient outcomes, satisfaction, and experience.
7. Seniors requiring palliative care must undergo a preliminary assessment conducted by our medical team to determine their suitability for residency. (CBC etc)

Responsibilities:
- Sukhada Team: Responsible for providing comprehensive palliative care services, including consultation, care planning, and symptom management.
- Admitting Physicians: Responsible for screening Seniors for palliative care needs and referring them to Sukhada Team as needed.
- Nursing Staff: Responsible for implementing the care plan and providing ongoing care and support to Seniors and their families.
- Social Work and Spiritual Care: Responsible for providing emotional and spiritual support to Seniors and their families.

Monitoring and Evaluation:
Sukhada Team policy will be monitored and evaluated regularly to ensure that it is effective in providing high-quality palliative care services.
The evaluation will include:
- Patient satisfaction surveys
- Family satisfaction surveys
- Staff satisfaction surveys
- Review of patient outcomes, including pain and symptom management
- Review of care plan implementation and effectiveness

The Package includes:
• Separate Private Room with AC, fridge, TV
• Food includes (Breakfast, snacks, lunch, tea & snack and dinner with soup & dessert)
• Weekly Doctors visit
• Dietician visit
• Yoga / meditation sessions
• Physiotherapy sessions
• Activity / Interaction: (Small outings, Singing, Birthday celebration, Festival celebrations)
• Separate washroom with toiletries

The Package Excludes:
• Medicines
• Investigation Medical Procedure during the stay
• Clothes and Accessories

Note: Once the Package amount is paid, it will not be refunded in any case.
However, in any circumstances Seniors wants to withdraw the stay then Deduction between 1 to 8 days: 75% to 80% on total package & between 9 to 15 days: 25% to 20% on the total stay.
